Virtual Nursing and AI Remote Monitoring

In a typical virtual nursing model, experienced RNs work from a central hub or from home. They connect to patient rooms through a camera, microphone and speaker, often built into the room's TV or a wall-mounted unit, and they have full EHR access. The virtual nurse is part of the care team for a group of patients and takes on tasks that do not need hands. These include admission histories and assessments of history, medication reconciliation, discharge education and follow-up instructions, rounding check-ins, answering patient questions, mentoring less experienced nurses, and, where policy allows, serving as the second verifier for high-alert medications. Virtual nursing is different from two related models. Continuous virtual observation, or virtual sitting, uses trained observers who may not be nurses. They watch many patients at once for falls, elopement or line pulling. Tele-ICU uses remote critical care nurses and intensivists to monitor ICU patients and has been in use for about two decades. Mercy Virtual Care Center in Chesterfield, Missouri, opened in 2015 as a dedicated telehealth building with no patient beds, and it is often cited as an early large-scale example. AI appears in several places. Computer vision detects bed exits, falls and room entries. Worklists rank which patients the virtual nurse should see next. Deterioration models flag patients for a closer look, and ambient tools draft documentation. One misconception is that virtual nurses count toward bedside staffing. Most models are designed as extra support. When hospitals use them to justify higher bedside patient loads, nurses and unions object. A virtual nurse cannot turn a patient, start an IV or respond physically to an emergency. Patient privacy also needs care. Cameras are usually off or covered until activated, with a chime or indicator light when someone connects.
