A 2023 systematic review and meta-analysis found small effects on anxiety and agitation and narrative evidence of increased sociability, but rated the evidence low quality because studies were methodologically limited and confidence intervals wide. The findings do not establish equivalence to animal-assisted therapy or guarantee benefit for an individual resident. Newer conversational AI companions go further, engaging in short dialogues, playing music, or facilitating reminiscence therapy by discussing old photos or life events, a technique with established benefits for dementia patients' mood and identity. Some tools use natural language processing to detect distress in a user's speech and offer calming responses. The central ethical debate is around deception: does it cause harm to let a person with dementia believe a robotic pet is alive, or to have an AI voice mimic a deceased relative? One ethical debate is whether a comforting simulated interaction may reduce distress or instead undermine autonomy, dignity, and trust. A person’s preferences and assent should guide use; a diagnosis alone does not establish consent. Any test involving a simulated deceased relative should have ethics review, privacy safeguards, and an option to stop. A common misconception is that these tools are meant to replace caregivers; the evidence and design intent both position them as supplements to, not substitutes for, human contact.

Use of simple sensor-based companions like PARO is likely to keep expanding in care facilities given consistent evidence of reduced agitation, while conversational AI companions with generative dialogue remain earlier stage and more ethically contested, especially around simulated-voice products. Expect continued debate and likely regulatory or ethical-guideline development around consent and disclosure standards rather than a settled consensus in the near term. Broader clinical trials will likely shape which specific use cases become standard practice. More rigorous and longer-term studies should separate effects by device, care setting, dementia stage, and outcome. Any deployment should preserve human contact, privacy, the person’s dignity, and a clear option to decline; companionship technology is not a substitute for care.
