ICD-10-CM, CPT and HCPCS represent different coding systems. A single encounter may require multiple codes for documented diagnoses and procedures, which is one reason automated coding is modeled as multi-label prediction. CMS guidelines tie code assignment to provider documentation and include sequencing and specificity conventions. HCPCS Level II codes identify products, supplies and services not included in CPT. A proposed AI code should be verified against the chart and applicable guidance; the cited vendor site does not establish autonomous radiology coding with minimal review. An inaccurate code can lead to claim denial, correction or repayment. The False Claims Act requires knowing falsity for liability; an error by itself does not prove a knowing false claim.
