It matters because paperwork is a major driver of clinician burnout and capped caseloads, and every task handed to AI still has to meet privacy rules such as HIPAA and remains the clinician's legal responsibility.
Most private practices already run on a practice management system such as SimplePractice, TherapyNotes or Jane, which bundles calendars, intake forms, telehealth, billing and records. AI now sits on top of that stack in four main places. Scheduling tools automate reminders, waitlists and self-booking. Intake tools pre-fill forms, route screening questionnaires and summarize long histories. Billing tools check eligibility, scrub claims for coding errors and help sort out denials. Marketing is the lowest-risk use: drafting website copy, directory bios and newsletters that contain no client data. The legal frame is HIPAA. Any vendor that creates, receives or stores protected health information on a practice's behalf is a business associate and must sign a business associate agreement (BAA). Consumer chatbot accounts generally do not come with one, so pasting session details into them is a privacy violation waiting to happen. For self-pay and uninsured clients, US practices must also provide a Good Faith Estimate of expected charges under the No Surprises Act, which software can generate but the clinician must check. Two misconceptions cause most trouble. The first is that deleting a client's name makes text anonymous. HIPAA's Safe Harbor method lists 18 identifier types, including dates and geographic details, and a rare life event can identify someone on its own. The second is that AI billing or notes shift responsibility. The clinician who signs a note or submits a claim is accountable for its accuracy, so a fabricated symptom or an upcoded service in an AI draft is the clinician's problem, not the vendor's. Recording sessions also requires informed client consent, and some states add stricter rules.

Expect AI features to keep moving inside the practice management systems clinicians already use, which cuts down on copying data between tools but concentrates a lot of sensitive data with fewer vendors. Payers are also adopting automation on their side, so claim and prior authorization exchanges may increasingly be software talking to software. Professional associations and licensing boards are still working out guidance on recording sessions, client consent and how to disclose AI-drafted documentation. Clinicians who document their vendor choices, keep BAAs on file and review every draft will be best placed as those rules settle.
