AI can suggest a reflection prompt, ask one follow-up question or organize notes you have already written. If you want to try a thought record, a common CBT exercise, write down the situation, initial feelings, thought, evidence for and against it, and a more balanced or neutral alternative. NHS guidance presents this as a structured self-help exercise. Balance matters. Reframing is not pretending a real problem is harmless. If the stress comes from an unsafe situation, discrimination, financial pressure or an urgent decision, practical support or action may be more appropriate than changing the wording of a thought. You can ask the model to help list options or questions for a counselor, but verify factual information and do not let it discourage help-seeking. If writing makes you feel worse, stop and reach out to someone you trust or a qualified professional. Protect your privacy. Emotional journals can contain sensitive information; do not assume a general chatbot will keep entries confidential or fall under health privacy rules. You can use AI only to generate prompts and keep the writing in a private notebook, or use an approved tool with settings you understand. Follow school or workplace policy if applicable. If you may harm yourself or someone else, or are in immediate danger, contact local emergency services or crisis support instead of continuing the chat. In the United States and territories, call or text 988 for mental-health crisis support.

Mental-wellbeing products may add more journaling and reflection features, but their effects and safeguards need careful evaluation. WHO experts have called for safety, accountability and evidence in AI used around mental health. Until tools are designed and tested for that purpose, treat a chatbot as an optional writing aid and keep clinicians, trusted people and crisis services in reach. People should not have to use AI to access support, and a product’s availability does not mean it is clinically evaluated. Schools and organizations can make human support routes clear and help users decide when private writing should stay offline.

Use a thought-record worksheet to list the situation, feelings, automatic thought, evidence on both sides and a more balanced alternative.

Ask AI to turn your own notes into questions for a therapist or counselor, without asking it to label a disorder.

If journaling increases distress, pause and contact a trusted person or qualified support rather than asking the chatbot to manage a crisis.
