Two misconceptions cause most trouble. The first is that deleting a client's name makes text anonymous. HIPAA's Safe Harbor method lists 18 identifier types, including dates and geographic details, and a rare life event can identify someone on its own. The second is that AI billing or notes shift responsibility. The clinician who signs a note or submits a claim is accountable for its accuracy, so a fabricated symptom or an upcoded service in an AI draft is the clinician's problem, not the vendor's. Recording sessions also requires informed client consent, and some states add stricter rules.
