Mental health chatbots vary widely. Some use scripted decision trees and structured CBT exercises, while others generate more open-ended conversational responses. Common features include mood check-ins, psychoeducation, activity planning, thought records, and reminders. The label “therapy chatbot” can overstate what a particular product does; inspect its clinical scope, human support, and evidence rather than assuming it delivers psychotherapy. Research on specific apps provides evidence about those products and study conditions, not every chatbot. For example, an early randomized trial of Woebot enrolled 70 adults aged 18 to 28 and compared a two-week CBT-oriented self-help chatbot with an information-only ebook. The trial reported a reduction in self-reported depression symptoms in the chatbot group, while anxiety changes were observed among completers in both groups. The study was short and unblinded, recruited a nonclinical college population, and included an author affiliated with Woebot Labs. It does not establish effectiveness for other ages, conditions, or long-term care. A chatbot can be useful for structured practice or reflection, but it cannot reliably assess every risk, understand all context, or provide the continuity and accountability of a clinician. It should direct users to qualified help when needs exceed its scope. People in immediate danger or crisis should contact local emergency or crisis services rather than wait for a chatbot response. Privacy matters because conversations may include diagnoses, trauma, medications, or crisis disclosures. Review how data are stored, used, shared, and deleted. Do not assume a consumer app is covered by the same rules as a clinical provider. Product status and regulatory claims vary, so verify current documentation and do not infer approval from marketing language. Use a chatbot as an optional support tool, not a diagnosis or treatment decision-maker. Evaluate its evidence, limitations, escalation process, and fit with the person's care needs. Professional judgment remains essential for treatment, medication, and crisis decisions.

Better safety monitoring can support clearer escalation, but fluent conversation alone does not demonstrate therapeutic benefit. Researchers should test specific interventions with appropriate comparators and longer follow-up. Users and clinicians will still need transparent privacy practices and professional care pathways. New models may make conversation more flexible, but clinical evidence must be gathered for each intervention. Apps should communicate boundaries and protect sensitive records as features change.
