Do not upload identifiable health records to an unapproved service. AI should not be used to decide that severe or rapidly worsening symptoms can wait; follow emergency or clinician instructions. You may use one question at a time if that helps you organize notes, but choose whatever format lets you record accurate details. Bring the original reports or portal access when relevant. The goal is to help you communicate, not make the visit a test of an AI answer.
