Annex IV is the technical-documentation content referenced by Article 11. When the relevant requirements apply, providers of high-risk AI systems must prepare the file before placing the system on the market or putting it into service, keep it up to date, and make it clear enough for authorities and notified bodies to assess compliance. The 2026 consolidated Act allows eligible SMEs, including start-ups, and small mid-cap companies to provide Annex IV information in a simplified manner using a Commission form; this does not remove the underlying requirements. The Annex groups information into nine areas. These include a general system description; design, development, and data details; system monitoring, performance, and control; suitability of performance metrics; risk management; lifecycle changes; standards and alternative solutions; the EU declaration of conformity; and post-market monitoring. The exact content depends on the system. For example, a product-embedded system may require one technical file that also includes documentation required by the relevant product legislation. Treat the file as a maintained evidence set, not a one-time report. Link intended purpose and system versions to validation results, datasets, risk controls, human-oversight measures, and deployment instructions. If a provider changes a model, training data, or intended use, record the change and assess whether existing tests and conformity evidence remain valid. Keep test reports dated and attributable to responsible people. Check the applicable high-risk category and amended application dates before deciding when a particular obligation applies. Under the 2026 Omnibus, the Chapter III high-risk requirements apply from 2 December 2027 to Annex III systems and from 2 August 2028 to Annex I product systems. ISO 9001 or ISO/IEC 42001 can provide useful management practices, but certification alone does not show that every Annex IV item has been covered. Map each legal element to evidence for the actual system and current version.

The Commission may update Annex IV or publish supporting forms as implementation develops. Providers should track the applicable consolidated text, harmonized standards and official templates before each compliance milestone. Keep revision histories that connect the technical file to the system version, validation results and responsible reviewers. When considering the simplified route, verify that the Commission has issued the required form and that the provider is eligible. A change to a model, data pipeline or intended use should prompt a documented review of whether existing evidence remains adequate.
A provider documents a proctoring system’s intended purpose, version, hardware, interfaces, and user interface to establish the basic system description.
A credit-scoring provider records training-data provenance, selection, cleaning, labels, validation data, and dated test reports.
A provider lists harmonized standards it applies and describes alternative technical solutions for any requirements not covered by those standards.
An eligible small company checks the current Annex IV simplified-documentation form and records which documents and evidence still apply to its high-risk system.
