Example

A content-moderation tool flags posts that may break the rules, but a human moderator makes the final removal decision on borderline cases. The moderator's decisions are fed back to improve the model.

Why it matters

People catch errors and handle unusual cases that models miss, and they keep accountability with a person. The design must make review real, not a rubber stamp.

Common questions

Does human review fix all errors?

No. People can get tired or over-trust the AI, so review works best when reviewers have time, context, and the authority to override.

When is it most important?

When errors are costly or hard to reverse, such as in medical, legal, financial, or safety-related decisions.

Is human-in-the-loop the same as human-on-the-loop?

Not quite. In-the-loop means a person approves actions; on-the-loop means a person supervises and can step in.
